This Entry and Non-Disclosure Agreement (“Agreement”) is entered into as of the date it is last signed by all parties (“Effective Date”) by and between Qwest Corporation d/b/a CenturyLink QC (“CenturyLink”), and the undersigned (“Interested Party”). CenturyLink and Interested Party may sometimes be referred to in this Agreement individually as a “party” or collectively as the “parties.”
