This EpicCare Link Access Agreement ("Agreement"), effective as of , 20 (the "Effective Date"), is entered into by and between the Board of Trustees of Hendricks County Hospital doing business as Hendricks Regional Health an Indiana county hospital ("HRH") and the undersigned ("Outside Entity") a licensed health care provider. HRH and Outside Entity are sometimes referred to in this Agreement individually as a "Party" and collectively, as the "Parties."
