The College of Arts & Sciences (CAS) of The University of Montana, Missoula has approved the delivery of the equipment listed above to you for your exclusive use on University of Montana business, within your duties as a CAS employee. The equipment remains University property for which you are responsible to safeguard and prevent from being damaged or lost. By accepting this equipment for use, you agree to take all reasonable actions to care for it. Should the equipment be misused or damaged beyond that attributable to normal wear and tear, lost, or not returned by the date designated herein, you may be held liable for replacement cost and may be charged the appropriate replacement cost at the time it becomes unusable. Should the equipment be stolen and you are able to justify to the satisfaction of the CAS Dean that neglect was not attributable to you, you will not be held liable for replacement costs.

VHAT-12's equipment is the property of the City of Verona. Equipment may be used only by residents of the City of Verona or members of non-profit organizations providing service to the City of Verona for producing programs to be cablecast on VHAT-12, Verona Hometown Access Television. Borrowers under the age of 18 will need a parent or legal guardian to co-sign. Proof of residency and age must be presented to the Communications Coordinator prior to check out. They are to be used for scholarship, research, teaching, service, and administrative purposes that are conducted under University auspices. Please be advised to secure any sensitive data according to FERPA or HIPPA standards and UW policies and adhere to the UW Tacoma “Data Protection Policy for Portable Devices”. By signing below, Borrower hereby agrees to use equipment in a safe and careful manner. Borrower agrees to pay the cost of repairing the equipment if damage to the equipment and/or its parts occurs due to an external cause or due to any negligence or any factor under the control of the Borrower. Borrower agrees to replace any equipment not returned to the City of Van Meter (the “City”). Borrower acknowledges and agrees that the City is providing the equipment only.
