The undersigned ("Tenant") has participated in the City of Philadelphia’s Eviction Diversion Program (the "EDP") with respect to a residential lease (the "Lease") entered into by Tenant, as tenant, and ("Landlord"), as landlord, for premises located at , Philadelphia, Pennsylvania (the "Property"). In connection with the EDP, Landlord and Tenant have agreed to, among other things, the settlement of past-due rent owed by Tenant to Landlord.
