This Brokerage Services Contract (Contract) is made by and between the Mississippi Department of Finance and Administration (MDFA), acting administratively on behalf of the Board of Trustees (Board) of the Mississippi State Agencies Self-Insured Workers’ Compensation Trust (Trust), and [Insert Company Name] (Contractor), effective February 1, 2022, under the following terms and conditions under which the Contractor agrees to provide excess workers’ compensation insurance brokerage services to the Board relating to the Trust.
