This agreement (the “Agreement”) between you (the “Field Examiner(s)”) and PRO- ED, Inc. (“PRO-ED”) is entered into upon your acceptance as indicated below (the “Effective Date”). This Agreement confirms the terms and conditions whereby the Field Examiner agrees to undertake the testing and/or coordination procedures for the collection of data related to certain tests (the “Test(s)”) agreed by the Field Examiner and PRO-ED within the Test database (the “Field Examiner Portal”) and access to Examinee information in the portal (the “Examinee Portal” or “Portal”). In consideration of Field Examiner administering the respective PRO-ED exam, providing the Contracted Data and PRO-ED providing the exam and payment to Field Examiner, the Parties hereby agree as follows:
