This responsibility includes key cards, and any damage to village property due to negligence on the part of any person covered by this membership. I also acknowledge that no one under the age of 18 is allowed in the fitness center without a parent or guardian and that only the people listed on this membership form are allowed to access the fitness center. There are no guests allowed.
