For fixed term rental agreements with a specified ending date that falls within the first year of tenancy, Eugene Code Section 8.425(17)(b) requires that landlords provide tenants with a written statement at least 90 days prior to the ending date of the lease. The written statement must inform tenants of the requirements related to tenants’ eligibility for relocation assistance. Landlords may use this form to inform tenants of those requirements and must include the attached description of tenants’ relocation assistance rights and obligations. This document is not a substitute for, nor is it intended to address, any additional legal requirements of the Eugene Code or state law.

This Fixed Term Rental Agreement (the “Agreement”) is effective the date specified below, and is made by and between Jim Luo (hereinafter, “Lessor”) and the person named on the booking for the Property via the booking platform (“Lessee”) for rental of one of the units located at the address in San Francisco as indicated on the reservation received by Lessor from the booking platform (the “Property”). The Property only includes the unit specified on the listing on the booking platform and does not include any common areas or any other units at the same address.
