By signing below, I am indicating that I have read and understand the FERPA Release and the SVSU Fraternity and Sorority Anti-Hazing Policy as printed on the back of this card. My signature indicates my consent to release my student records as outlined in the policy and that I understand that I may be held accountable to the SVSU Anti-Hazing Policy as both a new member and initiated member if I fail to report any violations of the anti-hazing policy.
