This FY 2022 Social Services Agreement (the “Agreement”) is made and entered into as of the date last signed by the parties below (“Effective Date”), by and between CASA OF FORSYTH COUNTY, INC., a Georgia nonprofit corporation, having a principal office address of 3250 Keith Bridge Rd Cumming, Georgia, 30041, (hereinafter “Agency”) and FORSYTH COUNTY, GEORGIA, a political subdivision of the State of Georgia, acting by and through its governing authority, the Forsyth County Board of Commissioners (hereinafter “Forsyth County” or “County”), collectively referred to herein as the “Parties”.
