Temporary service(s) being requested are authorized for this address only and limited for the use of construction purposes only. This request applies to temporary supply and use power in part of an electric installation or temporary connection of the plumbing equipment to the source of energy or fuel for the purpose of testing the equipment. Termination of approval for the temporary services will be discontinued upon any violation of the provisions of the applicable codes.
