This AGREEMENT (the “Agreement”) is made and entered into effective as of this # day of DATE, 201_ by and between the City of Phoenix, a municipal corporation of the State of Arizona (hereinafter referred to as “City”) and CONSULTANT, a STATE corporation with offices located at FULLADDRESS (hereinafter referred to as “Consultant”). As used herein either the City or the Consultant may be referred to individually as a “party” or collectively as the “parties”.
