This Amwell Group Practice Agreement (“Agreement”) is a binding document between you (meaning the individual person or the entity that the individual represents that has obtained the Service (as defined below)) (“You” or the “Customer”) and American Well Corporation, a Delaware corporation with a principal place of business at 75 State Street, 26th Floor, Boston, Massachusetts 02109 (“American Well”).
