In consideration of the Application made by the Group, which is part of this Contract, and in consideration of payment by the Group of the appropriate premiums, VisionCare Plan (Hereinafter called Plan) shall provide a provider network to perform services for eligible persons as defined by Group, subject to the terms and conditions of this Contract.
