By signing of this Sales Order, I accept responsibility for the items listed above. I individually contract and agree to guarantee to Tanglewood Medical Supplies (“TMS”), the compliance and performance of the item agreement with TMS signed by Customer/Guarantor, as evidenced by my endorsement of the contract. If I or my insurance fail to make the payments agreed to, fails to protect and preserve the equipment, or fails to return the equipment or items to TMS at the termination or upon breach of the agreement, I will personally be responsible to return the medical items to TMS at 2445-B Loop Stephenville, TX 76401, or for the monthly agreement payments and/or cost of the item/s upon failure to return. I further guarantee and promise to pay all charges incurred within thirty (30) days of written demand from TMS to me at my residence.
