This Health Care Medical Services Agreement (this “Agreement”) is made and entered into to be effective as of this 18th day of January 2022, by and between School Board of Gilchrist County, Florida (“Client”), with principal offices at 310 NW 11th Ave, Trenton FL, 32693 and Medical Risk Solutions, LLC (“MRS”), a Florida limited liability corporation with principal offices at 2710 Rew Circle, Suite 200, Ocoee, FL 34761. Client and are jointly referred to herein as the “Parties.”
