THIS AGREEMENT is made and entered into as of the day of _ 202 by and between the HEALTHPASS INSURANCE TRUST, (”Trust”) with offices located at 112 West 34th Street, 18th Floor, New York, New York 10120, and the person or entity named on the signature page hereof (“Credentialed Agent”). Trust and Credentialed Agent may hereafter be referred to individually as a “Party” and collectively as the “Parties”.
