This AMENDMENT is entered into between the Area Agency on Aging of Palm Beach/Treasure Coast, Inc., hereinafter referred to as the “Agency”, and Okeechobee County Board of Commissioners/Okeechobee Senior Services, hereinafter referred to as the “Case Management Agency”, and collectively referred to as the “Parties”.
