The following Terms & Conditions (which together with the Agreement Form to which they are appended are collectively referred to as this “Agreement”) are the full and complete agreement with the Customer (the “Customer” being the name and signatory on the Summitt Home Protection Plan (“HPP”) Agreement Form who is the person entering into this Agreement with Summitt Home Services LP). If more than one person is named on the front of the Customer’s utility bill, the Customer understands that each person shall be deemed the “Customer” and each person has the authority to enter into this Agreement. Each person shall be individually liable, and all of the Customers are collectively, jointly, and severally liable, for all obligations, liabilities and indebtedness imposed on Customer by this Agreement.

This Home Protection Plan (“Agreement”) is between the provider, Home Warranty of the Midwest, Inc. (or, in Oklahoma only, First Home Warranty of the Midwest) P.O. Box 1, Rock Rapids, IA, 51246, (877)977-4949 (“We”, “Us”, and “Our”) and the named Agreement holder (“You” and “Your”).

This agreement (“Agreement”) is entered into between you the customer, and The Manchester Group, LLC, (dba “IGS Home Warranty”)

Eligibility: All Protected Lines and Protected Systems must be attached to or located in a Single Family home owned by you, and for outside line protection, you must own the land upon which your Single Family home is located. All Protected Lines and Protected Systems must have been installed by a licensed contractor according to the applicable building code, and they must be in use and in good working order, without any Leaks or Failures, when you enroll. It is solely your responsibility to have or obtain any right- of-way needed by the contractor(s) to repair or replace any Protected Lines. If you have multiple systems of the same type, each system must be enrolled to receive protection under this Agreement. We reserve the right to confirm eligibility while processing any claim.
