This HOMELINK Participating Provider Agreement for Wellcare of Kentucky (the “Agreement”) is made effective as of June 1, 2015 (the “Effective Date”), by and between HOMELINK, a division of VGM Group, Inc. (“HOMELINK”), (“Participating Provider” or “Provider”):
