HUMAN SERVICES PRACTICUM III (HUMS 495A) LEARNING AGREEMENT

The Learning Agreement identifies the agency activities in which the Human Services student will be participating during the semester. The specific activities are negotiated among the student, the Field Supervisor, and the Practicum Instructor and are designed to meet the learning goals for the course.

IOWA DEPARTMENT OF HUMAN SERVICES IOWA MEDICAID ENTERPRISE Iowa Medicaid Provider Agreement

This is a Provider Agreement for participation in Title XIX of the Social Security Act (as amended) and the State funded medical assistance program known as the Iowa Medicaid Program. This Agreement, effective on the date specified on the signature page of this document, is between the State of Iowa, Department of Human Services, hereafter called the Department and the undersigned [Provider, Group Provider and its members or Practitioner(s)] hereafter called the Provider. The operations management responsibility for the Iowa Medicaid Program is through the Iowa Medicaid Enterprise (IME). On its effective date, this Agreement supersedes and replaces any existing contracts between the parties related to the provision of health care goods and/or services to recipients of the Iowa Medicaid Program. The effective term of this Agreement shall be six (6) years from the date signed herein.

The fieldwork placement will consist of 125 hours to be completed by the last day of class for the semester. Human Services students must complete the hours throughout the semester (10.5 hours per week) for 12 weeks. This can be done either in-person or remotely. Micro-Internships or short-term remote project- based learning experiences may also count as a field placement. Human Services faculty must preapprove all remote Micro and Full Semester Internships.
