The following information is requested by TD Station Management to assist in the review and consideration of your request for a license to use TD Station facilities. It is very important that the information provided below is accurate and complete so your request can receive maximum consideration. Be as detailed and specific as possible. NO DATE WILL BE HELD UNTIL THE ICE USAGE AGREEMENT IS APPROVED AND PAID IN FULL. Until this application is reviewed by TD Station Management, there is no legal or binding commitment between TD Station and the applicant. Upon being reviewed by TD Station Management, the Applicant will have a license to use the TD Station facilities ON THE TERMS AND CONDITIONS SET OUT IN THE ATTACHED SCHEDULE "A".
