The Department of State Health Services (“DSHS” or “System Agency”) and Dallas County, Texas a political subdivision of the State of Texas on behalf of Dallas County Health and Human Services Department (“Grantee”), collectively referred to herein as the “Parties” to that certain Immunizations/Locals Grant Program DSHS Contract No. HHS000119700016, effective September 1, 2018 (the “Contract”), now want to further amend the Contract as reflected in this Amendment No. 2. The Contract was established under the Immunization/Locals (“IMM/LOCALS”) Grant Program.
