*This contract should be developed by the student, in conjunction with the supervising faculty member. The student should meet with the supervising faculty member to discuss, revise, and sign the contract as appropriate. A copy of the signed independent study contract should remain on file with the supervising faculty member and the department chair. Completed form should be sent to the Office of Academic Programs before the end of the semester add/drop period.
