Monarch Sky Lane, Suite 330, Meridian, ID 83646, and Northern Lights Fund Trust IV (the “Trust”, and together with Inspire, the “Parties”), on behalf of Inspire Fidelis Multi Factor ETF (the “ETF”), 225 Pictoria Drive, Suite 450, Cincinnati, OH 45246.
