This Individual Instructional Services Agreement (ISA) is made and entered into this 30th day of June, 2024, by and between Cerritos Community College District (“DISTRICT”) and a faculty-qualified nurse provided by PIH Health, Inc. (“INSTRUCTOR”) provided by PIH Health Whittier Hospital, PIH Health Downey Hospital and PIH Health Good Samaritan Hospital (collectively referred to as “PIH Health”), (together, “the Parties”).
