This MEMBERSHIP AGREEMENT (this “Agreement”) is effective as of the date below between Dymonte Thomas Sports Academy, LLC, a Colorado limited liability company (the “Sports Academy”) with an address of 8831 N. Harlan St., Westminster, CO 80031 and you, the athlete (the “Athlete,” or “Member”) or, if applicable your parent or legal guardian (“Parent or Guardian”) on the minor athlete’s behalf, with either of the above names interchangeably used on the agreement and account profile and in consideration of being permitted by the Sports Academy to train in its facilities or with its coaches or programs in any other venue where such training may occur, and the intangible value the Member will gain by training with the Sports Academy, and in recognition of the Sports Academy’s reliance hereon, Member, and Member’s Parent or Guardian as applicable, agrees to all the terms and conditions set forth in this Agreement.
