The purpose of this Agreement is to set forth the legal terms upon which Integrated Health Partners (“IHP”) may grant remote access to a Physician Entity, Group or Office (“Physician Office”) for the benefit of a Physician(s) and members of the office staff for Physicians who are a member of IHP. This Agreement should only be executed after IHP has received a request for remote access and has approved that request, pursuant to its policies and procedures. The parties acknowledge that IHP is an Organized Health Care Arrangement (“OHCA”) and that IHP grants this access to Physician Office consistent with IHP’s own restrictions and obligations under all applicable laws, rules, and regulations.
