This INFORMATION SHARING AND DATA USE AGREEMENT (this “Agreement”) is entered into by and between the Kansas Department of Health and Environment, located at 1000 SW Jackson Ave, Topeka, KS 66612 (the “KDHE”), and , located at (the “Provider”), concerning the provision by the Provider of a Data Set to KDHE to be submitted to the BioSense Program (as defined below) pursuant to the terms of this Agreement.
