THIS SERVICE AGREEMENT (the "Agreement") is made and entered into as of 2023 by and between Transformational Family Counseling PC, dba: S.U.P.E.R. Peer Counseling Program ™ located at 3400 Central Ave STE 215, Riverside, Ca 92506 (hereafter the "CONSULTANT") and Coachella Valley High School located at 83800 Airport Blvd, Thermal, CA 92274 (hereafter the "CLIENT").
