The following individual or company does not wish to have Dorick Navigation, S.A., insurance coverage applied automatically to the Dorick Navigation, S.A. Bill of Lading. By executing this document, I understand all limits of liability will be governed by the Carriage of Goods by Sea Act and the back of my Dorick Navigation, S.A. Bill of Lading. In no event will Betty K. Agencies (USA) LLC and Betty K Agencies Limited be liable to me for any loss or damage arising out of my election not to obtain insurance coverage, and I hereby release and hold Betty K Agencies (USA) LLC and Betty K Agencies Limited harmless of any liability howsoever caused. I also acknowledge future shipments will not be automatically insured.
