THIS AMENDMENT is entered into the effective dates specified below by and between Miami Dade Health & Rehabilitation Services, Inc. and Humana Medical Plan, Inc., PCA Health Plans of Florida, Inc., Family Health Plan, Inc., Humana Health Insurance Company of Florida, Inc., Humana Insurance Company, Employers Health Insurance Company and PCA Life Insurance Company an their affiliates who underwrite or administer health benefit plans (collectively, “HUMANA”).
