This Investigator-Initiated Clinical Trial Agreement (“Agreement”) is made as of this {DAY} day of {MONTH}, {YEAR} (the “Effective Date”) by and between THE RESEARCH FOUNDATION FOR THE STATE UNIVERSITY OF NEW YORK a nonprofit, educational corporation organized and existing under the laws of the State of New York, with an office located at Office of Sponsored Programs, Stony Brook, NY 11794-3362 at Stony Brook University (“Institution”) and {COMPANY NAME}, a corporation having its principal place of business at {COMPANY ADDRESS} (“Company”). Company and Institution are herein referred to collectively as “Parties.” Individually, each of Company and Institution is a “Party.”
