hereby acknowledge that we have received and reviewed with Player the LASC CODE OF CONDUCT CLUB AGREEMENT. I/We agree to abide by the Code of Conduct Guidelines to help LASC achieve its stated goals and objectives. I/We understand and agree that as a volunteer organization, LASC needs the active participation of each family and player if it is to successfully provide a positive atmosphere for players in development of their soccer skills and beneficial personality characteristics such as sportsmanship, dedication, commitment, respect for others, a strong work ethic and a sense of pride and self-worth.
