By signing this user agreement, I agree to accept responsibility for the return of all library materials, paying fines (including any applicable penalties and actual collection costs), paying for damage to or loss of materials charged on this card, notifying the Library of any change of name or address, and reporting if the card is lost or stolen. I understand that my failure to pay these fines when due may result in the balance being forwarded to a collection agency. I understand that this card is not transferable. I understand that overdue notices are sent as a courtesy, and that failing to receive an overdue notice does not absolve me from responsibility should I receive a bill or fine. I understand that borrowing limits and fine schedules are posted on the library’s website, and that any changes to these policies will also be posted on the library’s website https://ulib.iupui.edu.

This Agreement outlines the terms and conditions pertaining to the issuance by a requesting institution (the Requester) of Library Cards to specific investigators employed by that institution for the purpose of accessing controlled-access data through the National Human Genome Research Institute (NHGRI) Analysis, Visualization, and Informatics Lab-space (AnVIL) platform, an NIH-designated data repository. Controlled-access datasets are maintained in AnVIL per the expectations of the NIH Genomic Data Sharing Policy. All terms set forth in the NIH Data Use Certification (DUC) Agreement (except to the extent expressly modified herein) are hereby incorporated herein by reference and apply to future data access requests for datasets by Library Card Holders under this Agreement.
