This CINA Foundation Suite™ Licensed Software Agreement Purchased Through DARTNet Institute (the "Agreement") is entered into as of the date of the last signature hereto <<EFFECTIVE DATE>>, by and among <<LICENSEE>>(“Licensee”), with a principal place of business located at <<ADDRESS>>, and the DARTNet Institute (“DARTNet”), a Missouri non-profit corporation, with a principal address located at 11400 Tomahawk Creek Parkway, Leawood, KS 66211. Collectively, Licensee and DARTNet shall be known as the "Parties".

BY OPERATING THIS PRODUCT AND USING THE LICENSED SOFTWARE, YOU AGREE TO ALL THE TERMS OF THIS LICENSE AGREEMENT AND LIMITATION OF REMEDY PROVISIONS. IF YOU DO NOT AGREE WITH THE TERMS OF THIS LICENSE AGREEMENT, RETURN THE LICENSED SOFTWARE TO THE SELLER FOR A FULL REFUND.
