This UT Southwestern EpicCare Link Site Agreement (“Agreement”) is made and entered into as of by and between (“Outside Entity”) whose address is and The University of Texas Southwestern Medical Center, an agency of the State of Texas, located at 5323 Harry Hines Boulevard, Dallas, Texas 75390 (“UT Southwestern”). Outside Entity and UT Southwestern are collectively referred to as the "Parties" or individually as a "Party."
