Contract (the “Contract”) for the establishment of a Locked-In Retirement Account (the "LIRA") 574-620 between Computershare Trust Company of Canada (the “Trustee”), a trust company incorporated under the federal laws of Canada and having a place of business at 100 University Ave. – 8th Floor, Toronto Ont. M5J 2Y1 and the applicant (the “Annuitant”) named in the application for a BBS Securities Inc. Self-Directed Retirement Savings Plan, Declaration of Trust (the “RSP”), such application as found on the reverse side hereof or as attached hereto (the “Application”). The parties hereby agree for good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged by both parties hereto, as follows:
