This business agreement (hereinafter referred to as “Agreement”) is made and entered into by the Regional Lead Agency operating the Louisiana Services Network Data Consortium (hereafter called the “Regional LSNDC Lead Agency”), and [NAME OF PARTICIPATING AGENCY] (hereafter called the “Participating Agency”), an agency providing housing and/or support services to persons in the state of Louisiana within LA-509: Louisiana Balance of State CoC.
