HMIS assists the Philadelphia Continuum of Care to become better informed on the extent and nature of homelessness over time. Specifically, HMIS can be used to produce an unduplicated count of homeless persons, understand patterns of service use, and measure the effectiveness of homeless programs.

Contract Type FiledJune 12th, 2023For purpose of this agreement, the participating Homeless Service Provider will be referred to as "Participating Agency,” the Consumer of Services as the "Client" and the San Joaquin Continuum of Care Homeless Management Information System as "SJCoC HMIS." Clarity is a web-based client information system, used by the SJCoC HMIS to record and track homeless client information. It can also be used for case management, determining utilization of services of participating agencies, and sharing of information on services provided to homeless clients.
