This Second Amendment for Supplemental Services to Master Agreement for Safe Haven Sites and Services (“Second Amendment”) is effective as of July 19, 2021 (“Effective Date”) and is entered into by and between the Board of Education of the City of Chicago, a body politic and corporate, commonly known as Chicago Public Schools (“Board” or “CPS”), and Greater St. John Bible Church with offices located 1256 N. Waller Ave., Chicago, IL 60651 (“Provider”).
