THIS AGREEMENT, made and entered into effective as of the date a signed Service Order (“SO”) or Master Service Agreement Addendum (“Addendum”), by and between “Customer,” and Douglas Services, Inc. dba Douglas Fast Net (DFN), an Oregon company, hereinafter referred to as “DFN”. This Agreement becomes legally binding upon signature by both Customer and DFN.
