This Media Permission Form (the “Agreement”) will serve as our agreement concerning your participation at the Conference. In consideration for the speaking, and/or performing, opportunity provided to you by TEDMED, you grant TEDMED and TED Conferences, LLC (“TED”), and other entities — e.g., broadcasters, internet sites — as TEDMED may designate from time to time, the right to record, film and photograph your presentation at the Conference (the “Presentation”) and to distribute, broadcast, televise, translate or otherwise disseminate it, without any further approval from you, in whole or in part, throughout the world, in perpetuity, in any and all media now known or hereafter developed. This grant to TEDMED and TED includes, but is not limited to, the right to edit the Presentation, and the right to use the Presentation either alone or together with supporting information, such as your name, voice, photograph, likeness and biographical data (collectively, “Supporting Information”). Macalester College would like to produce a limited broadcast, keep an electronic record of your presentation and invite electronic and other media to the event. Macalester College recognizes your rights to the material and this agreement constitutes a license to the extent identified. Rotary District 6960 is planning to take photographs and video of individuals and activities during the RYLA weekend event. We are hoping to produce print and video materials that we will use to promote RYLA in our District and throughout the Rotary world. In order to create such a production we need your permission to take the photographs prior to the event. We may also give each student a Directory with bio information on the weekend’s participants so that the RYLA Delegate can stay connected after the event.
