Failure to sign this form will result in your child(ren) being unable to participate in the WWEST/iSTAND hosted camp, and the forfeit of fees. Please note that WWEST/iSTAND camps continue to follow COVID-19 protocols and regulations as mandated by the Federal, Provincial and/or Local Health Authorities, as well as the University of British Columbia, Okanagan (UBCO). Any participants exhibiting related symptoms will be required to withdraw from camp to ensure the safety of our staff and other campers.

By signing below and participating in any event, photo shoot, video session or other activity in connection with Onnit Labs, Inc. (“Onnit”), whether promotional, commercial or otherwise, I hereby grant to Onnit and its legal representatives, parents, subsidiaries, affiliates, successors and assigns, the irrevocable and unrestricted right, but not the obligation, to use and publish photographs, videos, my likeness, voice and/or any other media of me or in which I may be included, for editorial, trade, advertising, and any other purpose (collectively “Media”), and in any manner and medium; and to alter and composite Media without restriction and without my inspection or approval.
