MEDICAL CHARGE ACCOUNT AGREEMENT AndMedical Charge Account Agreement • August 28th, 2019
This Medical Charge Account Agreement and Initial Disclosure Statement (the "Agreement") governs the terms and conditions of your AccessOne® Medical Charge Account sponsored by the participating healthcare provider named on the front of your card (the “Provider”). Your Provider account and all rights to it will be assigned immediately following the execution of this agreement to AccessOne, a North Carolina corporation ("AccessOne"). The words "you" and "your" refer to the Account Holder. "We," "our," and "us" refer to AccessOne and to any other creditor to which this Agreement is assigned. At your request, we will issue an AccessOne Medical Charge card (the “Card”) and will establish a revolving charge account (the "Account") in your name, to which charges may be posted for healthcare services, equipment, and supplies which are rendered or sold to you ("Purchases") by the Provider not paid in full at the time of service, discharge, or sale, whichever is appropriate.
