This agreement is entered into between _________________ and the _________________ for the purposes of establishing an interdisciplinary collaboration called the ________________ to support _________ (Medical Partner)’s mission to deliver excellence in medical care to improve the health status of our region, while providing vital services to those in need to and training health professionals through affiliation with __________ (academic partners where appropriate). This agreement supports a multi-disciplinary, collaborative approach to health care and legal services delivery, which considers simultaneously the medical, social, economic, and environmental determinants of patients’ health and promotes the role of interdisciplinary education in serving patients’ health needs effectively. The collaboration has three primary, aspirational objectives:
