This Membership Agreement (the “Agreement”) is entered into by and between the undersigned patient (the “Patient” or “You”) and Redrose Medical (the “Practice”), a medical services provider. This Agreement outlines the terms and conditions under which the Patient agrees to participate in the Redrose Medical Membership Program (the “Program”) offered by the Practice. By providing the necessary information, signing this document, and paying the Annual Membership Fee, the Patient agrees to be legally bound by the terms of this Agreement.
