This Waiver Form (“Agreement”) is entered into by the undersigned (“Volunteer”) and the global medical mission organization (“MTIMA”). This Agreement governs the Volunteer’s participation in the medical mission trip from Canada to Malawi. By signing this Agreement, the Volunteer acknowledges and consents to the terms and conditions outlined herein.
