This Amendment (“Amendment No. 1”) to Contract No. MA-042-22011309 for Medical Transportation Services is made and entered into on July 1, 2022 (“Effective Date”) between Viewpoint Ambulance (“Contractor”), with a place of business at, 1341 N. Miller St., Anaheim, CA 92806 and the County of Orange, a political subdivision of the State of California (“County”), through its Health Care Agency, with a place of business at 405 W. 5th St., Ste. 600, Santa Ana, CA 92701. Contractor and County may sometimes be referred to individually as “Party” or collectively as “Parties”.

This Amendment is hereby entered into by and between Midwest Medical Transport Company, 2155 33rd Ave., Columbus, NE 68601 (hereinafter “Contractor”) and Lancaster County, (hereinafter “County”), for the purpose of amending the Contract dated May 31, 2016, under County Contract No. C- 16-0249, for Medical Transportation Services - Lancaster County Community Mental Health Crisis Center, Sole Source SS-35, which is made a part hereof by this reference.
