THIS AGREEMENT is made and entered into by the Kansas Department Of Health and Environment, Division of Health Care Finance (“The Department”), an administrative agency within the executive department of the State of Kansas, having its principal office at 900 SW Jackson, Suite 900 N, Topeka, Kansas 66612, and Health Plan Name (“Medicare Advantage Health Plan” or “MA Health Plan”), a corporation organized under the laws of the State of Kansas and having a principal place of business at Health Plan Address.
